Php email form

Hi!
I try to make a homepage and put a html & php email form.
The code is ok, but when I open my mail: I get this:

First Name: Peter
Last Name: Lars
Email: sew@yahoo.se
Telephone: 22222222
Family name:
Comments: sde

Family name is empty. I can not fint the error. please help me.

You want to receive the family name value but you pass the last_name2 value to the message;
