Background: I am working for a medical company that has three office. They all want patients to be able to pay their invoices online. All their patients billing information is handled by a separate company that has all three office's record in one huge database...yeah what a mess.
The Problem: The patient receive a paper invoice in the mailing telling that the they owe X amount to office A and Y amount to office B and Z amount to office C. Since the price the patient pays is variable I need a shopping cart utility where a patient can input the amount the want to pay to each office. Also I would like to use paypal as the merchant.
Solution: The general flow should be as follows...
1. Payer enters in patients name and account #
2. Payer selects what office they would like to pay and the amount they would like to pay each office
3. Payer fills out credit card info on paypal site and submits payment

If all 3 offices are owned by 1 company, why can't the customer just pay the combined total and let the company check the records and transfer the exact amounts to the 3 offices accounts? As long as the payer pays the full amount the 3 offices can be notified that the bill was paid. They'll just need to wait a day or 2 for their portion to be transferred from headquarters.
Or does it need to be possible that the payer can pay 100% of Office 1's fee, 25% of Office 2's fee, and not pay Office 3 at that time?

They need to specify to which office they are sending payment since each office handles its self as a different entity. Also think about how much would it suck to make someone's job to go through each invoice and separate out how much each office should get of the payment. What if the payer meant to pay 100% that he owes to office B, but only 10% of what he owes to office C?
I'm think about building a custom shopping cart with codeigniter's shopping cart class. Any thoughts?

I'm not that familiar with shopping carts, paypal,or codeigniter.
But I'm very interested to know if you can check out a shopping cart item ($100 - Office C Services) and at the paypal stage of the transaction enter $10 ("only 10% of what he owes to office C") and have it succeed.

I ended up using Paypals DoDiretPayment API and build my own custom shopping cart. Everything worked great so thank you for your responses. As response to an earlier question Paypal's integrated Shopping does not let you vary the price to items so don't worry Although you really should have someone manually checking every order to make sure it is correct. I can post code or share my shopping cart if anyone is interested.
