[Help] Image uploader get info and put it in file

Hi, I have a image uploader for my site, and I want to know how I would make it so that in the form it has boxes that ask for . Name, Team, and Date. Then when they enter it. It takes the image name and puts the info in a text doc. Like this

28371.jpeg - Name, Team, Date
17233.jpeg - Name, Team, Date

I got some of it figured out, but I can't get it fully.
