I Need an Email Signup Form for Client Sites

Greetings,
I have a service for merchants where they can send out discount offers to those who subscribe to receive these emails. My site will let the merchant log in and create the discount offer and then send it to those on their email list. What I need is to be able to give the merchant a link that they can put anywhere on their website that says something like "Click here to receive discount offer coupons by email". The link would then pop up an email sign up form, asking for the first name, last name and the email address. On submit, this would go to our site, where the confirmation email can be sent to the subscriber.
I want to simply provide the merchant with a link that leads to a popup, because this is a lot easier than trying to come up with a form that would need to be customized to fit where the merchant has space. Does anyone have any ideas as to what would be the best way to do this? My application is in PHP.
I have seen stuff like this using autoresponders like GetResponse.com.
