AutoResponse w/ jpg attachment

I want add an autoresponse w/ attachment function to my current php script.
The user fills out form and an autoresponse is sent with attachment according
to wich radio button they selected. Below is the current script I use.
Any thoughts appreciated. Will pay for time.
