Web application fault severity survery at Uni of Virginia

Hello,

My name is Laura and I am conducting a study on the severity of web application faults and failures at the University of Virginia department of computer science. Our goal is to estimate the distribution the severity of faults in real web application development environments. In doing so, we will be able to design testing techniques and methodologies that target high-severity faults.

If you are currently or recently developing such a product, I was hoping you would consider to take the survey. We are offering a drawing for a $25 Amazon.com gift certificate for survey participants. We do not record your name, company, or any other information that could identify your submission, therefore, the data we collect remains anonymous. The survey will collect the following information about faults that you have encountered during your web application development, in terms of the number of:

Level 0 faults: The fault was not noticeable by customers/users on the website/application
Level 1 faults: Customers/users would return this website/application again
Level 2 faults: Customers/users probably would return this website/application again
Level 3 faults: Customers/users would not return to this website/application again
Level 4 faults: Customers/users would complain about this website/application

Sorry, I'm not sure where to post it. I'm having a tough time finding developers to complete this survey. If you think it is inappropriate just remove it, I did not mean to offend anyone :(
