URLs with slashes (like Digg)

First off, sorry for my confusing title, I couldn't come up with anything that would explain what I mean...
Ok, here's my question:
Some sites like digg (http://www.digg.com) have urls like this: http://digg.com/general_sciences/How_to ... ucid_Dream
Is everything after the first slash processed by a script, or is that really just a file (without extension) within a folder?
If it is the first, how would one do that?
