PHP developer needed for simple checkout on small service web site.

This should be a tiny job, and I need to keep the cost low (this is actually a personal project out of my own pocket) but I'm a graphic designer with only a casual knowledge of PHP and little desire to get more serious. I am likely to have a lot more of these little tasks in the coming months and want a good, trusted person who will work at a nice cost and I can trust with access to my projects and clients. (I also need a good standby person experienced with X-Cart and ecommerce to help with a large project I'm sure we'll need assistance with).

As for this job... I simply need a checkout form that spans about 4 pages and gets billing info and CC info to me in a secure way. I would like the orders to be mailed to me, but it needs to be secure since there is CC info involved so open to another option. It does not need to authorize or capture from the cards. We'll be doing that manually (only 3-4 orders a day). I have a shared secure channel with my web server host. I will order a secure certificate. I can provide mySQL if needed. I can provide the pages with forms but I can't (or don't want to) make them work, and that's where you come in. It is my hope I can lay out the pages and turn them over to you to make the magic happen so I can take CC orders for the service we're offering.

It is also my hope this is only a 2 hour job if I turn it over to you with the databases ready and the forms laid out. I need it done by December 1, and I could give you the greenlight by next week.

More about the job:

This is for a small service web site. We offer two levels of a single consulting service, and a couple of add on options, so choosing those options needs to add cost to the order. I need customers to be able to supply credit card info and check out in a secure way, and I need these orders either mailed to me or a simple wb interface where my partner and I can look them up (perhaps being notified by mail when there is a new one?).

I would lay out the basic form pages and you could edit them as needed.

Ideally it would be something like this:

Page One - Customer chooses service and options and supplies name, address, the usual....

Page Two - Customer supplies Credit Card info....

Page Three - A verification screen that shows them all of the info gathered from the previous pages so they can be sure, and gives them a way to go back and correct. Perhaps the CC number can be XX'ed out on this page. There must be a checkbox on this page asking them to verify they read our terms and by checking it and submitting they go to....

Page Four - Thank you page.
