I once did an form that had about 70 fields in it. I put them all in one table. It was a one time record for a single individual.

typically if i have a data base with over 15 in it, i find myself putting the info into 2.. or 3.. or 6... nothing i've written to date required more than 15.. altho i did make one that ran 3 databases with about.. 13 in each, it was for my wifes daycare, and it had a really nifty looking flash front end, php back end.. and mysql super back end.
