Need two small forms created for Youth Group website.

Hello all!
I manage a small website devoted to a youth group of teens from 12-18 years old. I really want to incorporate a feedback form. I also have an idea for a small php application.
The application would be a simple 10 item form that can be updated with 10 different text sections. These sections are actually just 10 (up to 10) different activities that our being offered to our youth during that week. So, say Basketball - Tuesday - 7-9pm.. Soccer - Wednesday 6-8. The reason why i would really like an app like that is so a simple user can type in the information in the form field(s) and the app itself would format and post the info. The activities change almost daily so as the only volunteer 'webmaster' i am really starting to find it draining to constantly copy+paste text in basic HTML and format it then post each time.
Anyone up for this small project. The feedback form should just be a simple. Name field >> contents field>> with a mail function that captures and sends the info on submit.
