I'm not sure either, may be PEAR makes use of Sudo too

I don't know if it would work, but having a script that only retrieves the usernames of the file, and then having the suid bit set on that script would be less evil. (imho)

There is a slight difference between sudo-ing /bin/cat and /usr/local/bin/smbpasswdreader.sh though
