Flash and PHP

Hello,
I have a flash form located at:

»spec-works.com/flashlastproject/···Blue.swf

I have a PHP script that send the mail but I can figure out how to get it to attach a file.

If you click Attach File you can choose the file to attach but I need help with making it attached the file. In the STATUS and Alerts section it say attempting to upload the file.

Below is my php code:
