It doesn't matter if you do all the error checking in the world, or if you have the most beautiful graphics, if your site or application design isn't usable, it's not going to do well. Get input and advice on usability and user interface issues here.

I'm using a customised XAMPP (saved me from configuring everything myself) as a localhost.
Personally, I don't like the HTML Dreamweaver produces too much, but I sometimes use it for creating quick pages.

5: i have friends that are good with graphics do graphics. work together on deciding layout and such. obviously i do all the scripting, they make all the graphics0
mozilla & m$ie to make sure it's stanards compliant and ie people can access it too.
unfortunately this fails on cookies. for some reason ie people cant get cookies on a site that's running on linux.. well some of them. i can never recreate the issue.
