Mailer says sent but people don't get email

There are some pretty rare situations where system fails to send emails. This affects only one specific set of emails that our application sends (when we send email to people subscribed to specific object) - other situations work just fine (like welcome emails, reminders etc). Here is what I find in log (SwiftMailer 3.3.2_4):
Native mailer is used in this particular case. When we switch to SMTP than mailing features that used to work stop working completely. PHP 5.2.5...
Any pointers? Is this software or platform issue?
