Taking notes?

Taking notes?
I was wondering what approaches or systems people use to keep track of changes they've made or things they're working on.
I've found myself with many more websites and many of them are either complex or are often revised, and keeping track of what I've done or what I'm doing gets to be complicated!
For example, someone requests a new feature, and I create it and send it to them for approval, but don't hear back for a month or more. Then when they get back to me, I've forgotten what I did!
I've taken to keeping a basic daily log for each site in which I take detailed notes on what I did, what still needs to be done, etc. Seems to be working so far.
Any better ways to do this?
