Online User Site Testing Questionnaires?

Online User Site Testing Questionnaires?

Does anyone know a free online user-testing facility which allows for custom questionnaires? Purpose is to have users test a site and fill out the questionnaire.
Yes, I could design one, but I don't want to start yet another project and development cycle for that. So if you do know of one, please do let me know.

You could try one of those websites with the "how well do you know me" quizzes. They do nothing but make quizzes. However, it'd have to be hosted on their website, as they need the advertising money.

Google Custom Quiz. Has the one I mentioned, as well as a list of others at About.com.

Hmm, not quite what I am looking for - had a look earlier, but the quizzes I've found are multiple choice only, no textual entries possible...

Ohh... Thought multiple choice was what you were after. Just make a semi-contact form-ish kind of thing.
