At work now, I use Mantis fairly extensively. It has some nice points:

- Extremely common, so other systems (like test suites, planning, etc) have integration kits for it.
- Fairly stable and reliable
- Nice mail setup

It also has a few down sides:
- A bit cumbersome in its configuration and use of 'projects' as a categorization.
- Mail setup doesn't allow tagged subjects (to my knowledge, without substantial hacking) to facilitate easy sorting/filtering.

All in all, its nice. I've dealt with others (bugzilla, sourceforge's bug tracker, flyspray(?)), and it seems more comfortable than those.

Trac also has a bugtracker integrated with VC, etc, but it's a pain to get setup.

artic issue tracker (http://www.arctictracker.com/) is a nice solution, but it is all about what works for you. Some people still just use a spreadsheet, or google docs as it works well for them, others build bespoke solutions that intergrate directly with other internal systems.

Best free online bug tracker in my humble opinion is http://www.freebugbase.com . Been using it for about a year now and it works really well. I'm in the video game and website development industry and have used many others. This is the first I've found that works well an doesn't require an install.

I used to use Mantis. I used to like it but it's not that great any more. I've recently started using fixx from hedgehog lab. It's a lot better imo. The main thing I like about it is that it's browser based. It's not perfect though and I'd agree with som3on3 that the best bug tracker is a team of humans.
